I have gone several times to the convienience stores in Japan and I am frequently asked if I want chopsticks or if I want my meal warmed up. How do I respond in Japanese to refuse or accept? Polite and casual forms please!
 
I am guessing one would say something like Kekkou desu (結構です) as No thank you and Onegaishimasu (お願いします) as Yes please...

But don't take it from some guy who's never been to Japan

Well, there exist some ways to refuse and accept in polite manner on Japanese, but I'll you just the most used words in Japan say:

If you what at one shopkeeper/shop-member ask want, then so:

おはようでございます!/今日は!/今晩は!- O-hayou'degozaimasu/Konnichi wa!/Konban wa!//Good morning!/Good day!/Good evening!

あの。。。すみません。- Well... I'm sorry. - [Shopkeeper in this case says usual ->] いらっしゃいませ、何を。。。?/Irasshaimase, nan (w)o...?/Welcome, what you want...?

あれ/これをお願いします!/Are/Kore (w)o onegaishimasu!/This/That please! - [Shopkeeper...->] あれ?/Are?/That one?

ええ/いいえですよ。/Ee/Iie desu yo./Yes/No. [Shopkeeper...->] はい、どうぞ。/Hai, douzo./Yes, here are you.

感謝します、ありがとうでございます!/Kanshashimasu, arigatou'degozaimasu/I'll thanking, thanks! - いくらですか?/Ikura desuka?/How much is this?

[Shopkeeper...->] xxx円ですよ。/xxxen desu you/xxxYen (please).

わかりました、どうぞ。。。/Wakarimashita, douzo.../Did get, here then... - [Shopkeeper...->] ありがとうでございます!(Already mentioned...)

いいえ、いいえ。。。また会いましょう、さよなら!/...Mata aimashou, sayonara!/No, no... Also, goodbye, see you! - [Shopkeeper...->] さよなら!

"いいえですよ。"...? something seems off on that.

Just はい or いいえ would suffice. There is no need to affix です. A casual way of saying the two respectively would be うん and ううん (u'un rather than u-n as one extended syllable)

Sometimes "結構いいです" could be misinterpreted.

If you insert a pause between "結構" and "いいです", it's construed as "No thanks".
If you don't, it's construed as "Not bad at all".
