If you want to play it, i totally recommend go that route. The enemies are hard to kill, can give you status effects like poison, stun, etc and to level up you need to spend so much time.

Prologue
MC is captured and forced to live in Beriente with his wife Yuraha and daughter Seriha. The city has iron rules, separating men and women, they can't live together. Men are forced to work either as city guard (high class) or monster elimination squad (low class). Women work either as prostitute or work in farm where they are forced to work until they die without outside contact. MC is tasked to eliminate monster in dungeons, he is permitted to bring either Yuraha or Seriha to accompany him in battle. Who you choose will determine which route the game will follow. If you succeed all your missions, you and the family will be free to go. One of them must stay in the city as hostage to prevent them from escaping. There are some challenges you have to accomplished, faling to do so will incure consequences and will change the outcome of the game.

The story is well written, many mysteries i haven't gotten the answer yet maybe because i only finished 2 routes. MC is left oblivious until much later in the game. Sometimes the POV is switched to the heroine for the player to see what happens behind MC's back. The "challenges" are very good because you're given option to prevent "stuff" from happening. Some of the challenges are hard, few are easy.

Since i don't particularly like loli, I find it hard to choose Seriha route. Seeing your loli daughter in bleak dystopian city is not exactly pleasant. I love her character though. I wish there's option to turn off enemies encounter, especially if I've finished the game. Battles especially later in the game are simply too hard, it's like the author want to punish us to want to rescue MC's family.
No voice in the game. Many of the CGs are recycled from same picture but the author use zooming, bluring the images so you won't notice it. Women model in the game a bit too same-y, only different body, hair color.

I totally recommend this game if you have time to spare. avoid enemies at all times. Enter the dungeon, kill the boss, pick up items when you can. Use the password

Alright I am assuming you are talking about Please Bang My Wife and not Hoka no Otoko 3. If that is so I tried confirming your issue but sadly didn't find any problems. I can save just fine. Here is a screenshot of my own copy saving without much trouble.

I do get weird characters instead of legible English but that is a problem from MangaGamer's end as a lot of their customers complained the same. Not sure what the solution for that is but the game runs just fine even with that issue.

As far as how to save, there are 2 ways of doing that. One is from the interface at the bottom like in the screenshot here.

Things i love about the game

1. Even from the prologue, you kinda get what theme the game is trying to deliver. First you have to let innocent family to be killed simply because they know about the existence of your village. While you look away, your trash partner kills the father, son, and raping the mother. The city knows about your hidden village anyway and march the soldiers to the village. While you and Village rebel force are away doing mission to distract the City force, your family and other villagers are captured. You rush to your family only to see the women of the village are raped by City force. Your wife, Yuraha sacrifices herself for other women including her daughter. She is drugged and fondled by City soldiers. they try to make Yuraha admit that she wants it (btw the drug is experimental aphrodisiac, lol). Witnessing this from a far with binocular, you try to intervene. But your rebel force itself is divided between saving the villagers or saving themselves. Some of them are single without family. Some of them are also afraid that by intervening, they endanger their family in hostage. Hoping Yuraha alone can make for sacrifice. You intervene anyway but doing so most of villagers are killed and you and family are captured and taken back to Beriente.

2. Sacrifing others for your own happiness and avoiding things/not taking the matters in your hands only making it worse. This theme keeps recurring several times in the game. For example you have to fight in 4 colosseum. Winning or losing them will change the ending of the game. If you lose you have to present your most treasured thing, in this case Yuraha or your daughter Seriha to the winner. But it turns out your enemies are your former villagers that has been captured and turned into monster by experiment. You partially feel regret because it's because your intervention that they're killed/captured.
There's also flashback of your past life, how you've failed to do the right thing, making other people suffer instead. You as the character is a good person in conflict between duty and doing the right thing. When he tried to do both he actually make it worse. There's also other flashback of other character that'll reveal the mysteries about the world. sometimes you're also presented whether wasting time saving other people or hurrying to save your wife.

3. You can actually prevent some NTR scene from happening. Like i said above, you can waste time saving other people but in doing so you'll trigger the NTR flag. I still don't know whether all of NTR scene can be prevented though. There's also scene in dungeon where your wife is trapped with a villain and you have to figure out the password of the door.

4. The progression of the heroine and NTR scene progression. I love it when they show you what happen to the heroine while you try to progress in the dungeon. Like after several room progression you'll be presented what's happening on the other side (it doesn't happen every dungeon though). Also when you're separated from your wife in the dungeon, you get glimpse of what happen to your wife bit by bit. I also love it that in the late game your wife clearly shows different act toward you. She's in doubt whether getting away from the city is the best or simply staying and enjoying what she clearly likes, especially after certain arc.

In the near future? No. Why? Because Japan is anal about censoring. They are one of the most fucked up version of humanity when it comes to porn but they won't bend on their censoring policy. They even criminally penalized a lot of their own because their work did not have appropriate mosaic. So what does this have to do with localization? Two things. First, Japanese VN don't have enough fan following to warrant a solid investment. Only a handful of companies like JAST, MangaGamer etc. have taken the initiative but even they pick titles cautiously and serious NTR is one of the rarest genres. In-fact "Please Bang my Wife" is the only title that comes to my mind that has a streaming NTR theme from start to beginning with a English localization. Even fan works don't seriously target this genre.

In the far future? Sure, why not? I mean lets face it, even Steam is now endorsing visual novels. That is a game changer. A lot of Japanese companies will seriously consider the western market through Steam. This will give other companies and fan groups to target other games that Steam wouldn't pick, like a lot of extreme genre of eroge.

The only issue that comes to my mind is the problem of "reverse importing". We were once very close to having uncensored Japanese hentais couple of years back with Japanese companies actively partnering with western firms. But then Japanese consumers went, "OMG uncensored version! Me Want!". That effectively collapsed the Japanese censored products market with the influx of uncensored products. And since the companies couldn't challenge the Japanese mosaic law, they killed the initiative itself to keep the local market alive. This is what I fear might happen to visual novels as well. Been busy, but finally delved into Iroyoridori. Another great game from one of the leaders in NTR, and that's coming from someone that frequently dislikes a lot of the elements Jinsei games are built around(hetare profags, school-aged characters, lack of romantic relationship between the main couple) but the circle does all of the little things right 95% of the time, and that's worth experiencing. Also, they are amazing at moral degeneration/corruption.

Spoiler: Very minor spoilers

This time around, things are a bit lighter than before in some respects. The antagonist is a complete dudebro, and I loved him. If not for the fact that the profag is such a loser, he could easily be his actual buddy. I don't personally care for the protag, but that's typical for me in Jinsei games(the one from Honto no Kimochi was the only one even halfway decent) and it's not to say he's poorly written or anything, I just don't like weak-willed, indecisive pussies that only act masculine in the wrong instances.

There's a happy end, so folks looking for that can enjoy before or after they delve into the worse stuff. Unfortunately, there's very little mystery this time around. Even without the girl's perspectives, the profag witnesses most of what is going on(the only really important thing he misses is how Dudebro seduces the main girl, Mizuki)through videos or reading the Dudebro's conquest blog, and already knows exactly what kind of guy he is, having seen him 'steal' other friends and acquaintances at the school.

The circle is always improving; this time they do a very good job of showing the girls fall more realistically/less extremely. Dudebro is just a handsome, smooth-talker that is great at sex, and he makes the girls he seduces love sex, too. There aren't radical changes like in some of the previous games(though, you know me, I'm a bastard and love that shit), the girls start the game being sweet, kind-hearted, and lively, and end the game being sweet, kind-hearted, and lively while loving sex and Dudebro. Mizuki is a wonderful character and it's a treat to watch her turn sex-crazy. And it's a nice change from the usual tanned, tatted, drug-using turbo slut(but, of course, I love that, too).

This one won't be for everyone here, but give it a shot and I hope you won't be disappointed. I care most about the girls in these games, so I quite enjoyed it.

As an aside, Kabe 2 was not well-received by some here, but I enjoyed it thoroughly. Different strokes for different folks, so I'll avoid getting into a debate, but you should try it if you're interested. Anim has great production values if nothing else, and there are some amazing scenes in it. Kedamono is good stuff, as well, but definitely not to everyone's taste.
