In the subs you seriously did something wrong. There are a lot of errors like that as well...
Best of all: Two different spellings for the name of a character in the same amount of minutes - some things just shouldn't happen, especially if you did three versions for the TV and two for the BDs. I don't know about you, but shouldn't that mean five times QC'ing? It's a huge mess. >.<
